Qualifications

• High School Diploma preferred.
• Must complete one: A) training/competency evaluation as §418.76(b)(c); B) competency program meeting §418.76(c); C) state-approved nurse aide training/competency evaluation; D) state licensure program meeting requirements.

Responsibilities

• Provi​des direct patient care under RN supervision.
• Assists with transfers and ambulation per Hospice policy.
• Reports changes in condition and documents care provided.
• Maintains infection control and a clean, safe home environment.
• Communicates with patients, families, and staff with respect and clarity.
• Assists with nutrition and fluid intake.
• Supports efficient Hospice operations and time management.
• Performs treatments per Hospice policy.
