Job summary

Bristol Hospice in Green Bay, WI is seeking a compassionate Certified Home Health Aide (CHHA) to join our Interdisciplinary Group Team. The role supports patients under RN supervision, delivering personal care and comfort in the home setting.

Qualifications

• Must be a Certified Nursing Assistant (CNA) in good standing.
• California Regulation: Must be a CNA in good standing AND have a current HHA certification in CA.
• Preferred 1 year experience in community health/hospice or medical/surgical environments.
• Must have completed minimum 75 hours of classroom and supervised practical training.
• Licensed driver with insured automobile in good standing.
• Willingness to submit to a criminal background check before hire and annually thereafter.
• Current CPR certification.

Responsibilities

• Perform personal care and attend to patient requests.
• Provide patient and family with positive communication, confidentiality.
• Meet safety needs and use equipment safely.
• Provide personal care including baths, grooming, and changing bed linen.
• Assist in dressing/undressing patients as assigned.
• Plan and prepare nutritious meals and shop as assigned.
• Assist in feeding the patient.
• Take and record vital signs when ordered and demonstrate skills competency.
• Provide care to prevent tissue breakdown and monitor skin.
• Assess patient condition and report changes to Case Manager.
• Assist in ambulation and range of motion as instructed by the hospice team.
• Assist with transfers and positioning as directed.
• Assist with self-administered medications as allowed by state regulations.
• Offer bedpan/urinal aid and assist with basic household chores as needed.
• Provide respite for family/caregiver when on-site.
