Job summary

St. Croix Hospice in Minnesota is seeking a compassionate Hospice Aide (CNA) to join our in-home care team. You will provide hands-on personal care under an RN, ensuring comfort, dignity, and high-quality end-of-life support during 1:1 visits.

This full-time role requires CNA certification, a valid driver s license, reliable vehicle, and strong observation skills. Travel across patient homes, document care, and collaborate with the care team to uphold our mission of compassionate care.

Qualifications

• Current CNA certification in the state of practice.
• Valid driverb s license, clean driving record, and auto insurance.
• Record vital signs and report observations to the care team.

Responsibilities

• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing.
• Record vital signs and report observations to the care team.
• Assist with meal planning, preparation, and feeding as needed.
• Promote patient comfort, dignity, and safety.
• Travel to patient homes throughout the day and provide care as scheduled.
• Maintain accurate documentation of services provided.
