Job summary

Affinity Health Management is seeking a Hospice Aide to provide compassionate personal care to hospice patients under RN supervision. Responsibilities include bathing, feeding assistance, mobility support, and accurate documentation to support patient comfort and safety.

The role emphasizes respectful communication with patients and families, adherence to care plans, and reporting changes promptly to the RN Case Manager. A valid CNA credential and CPR certification are required.

Qualifications

• Completion of state approved CNA course or equivalent training.
• Nursing Assistant Certified (NAC).
• CPR certification (Healthcare Provider, hands-on).
• Food Handlers Permit.
• High school diploma or GED.

Responsibilities

• Perform personal care and attend to patient requests promptly.
• Provide patient and family with respectful, confidential communication.
• Ensure safety and proper use of equipment.
• Assist with bathing, grooming, feeding, and bed linen changes.
• Assist in dressing, transferring, and ambulation.
• Assist with medications reminders as allowed by state rules.
• Maintain clean, orderly living area and assist with light housekeeping.
• Document care and report changes to RN Case Manager.
• Participate in quality improvement activities.
• Adhere to care procedures and professional conduct.
