Job summary

Affinity Hospice Management LLC in North Carolina is seeking a certified nursing assistant to support hospice patients under RN supervision. You will perform personal care, assist with daily activities, and document observations per the plan of care to promote comfort and safety.

Responsibilities include communication with patients and families, maintaining privacy and confidentiality, and ensuring accurate reporting of any changes to the care team. A state CNA certification is required.

Qualifications

• Completion of a state-approved CNA course or equivalent training with current license.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understanding hospice philosophy and ability to provide specialized care to the terminally ill.

Responsibilities

• Perform personal care tasks and respond promptly to patient requests.
• Communicate with patient and family using respectful, confidential methods.
• Assist with safety needs and use equipment properly (foot stools, side rails, O2).
• Provide baths, back rubs, oral hygiene, shampoos, and bed linen changes.
• Assist in dressing, feeding, and comfort measures per plan of care.
• Monitor patient condition and report changes to RN Case Manager.
• Assist with ambulation, transfers, and range of motion as directed.
• Observe skin condition to prevent tissue breakdown and maintain cleanliness.
