Job summary

The Pennant Group, Inc. is seeking a hospice aide to support the interdisciplinary team under the supervision of a registered nurse. You will observe, document, and assist with activities of daily living to ensure comfort and safety for hospice patients in their homes or care settings.

Responsibilities include personal care, meal assistance, vital signs support, and adherence to care plans. Strong communication and empathy are essential for quality patient outcomes.

Qualifications

• Completion of state-approved hospice aide training and competency evaluation.
• Must be at least 18 years old.
• CPR certification required.
• Ability to drive and have access to transportation.

Responsibilities

• Perform personal care including baths, grooming and hygiene.
• Assist with meals, feeding and hydration as assigned.
• Observe patient and report significant changes to the Case Manager.
• Document care provided and adhere to procedures and standards.
• Assist with mobility and range of motion as directed by the nurse.
