Job summary

Qualifications

• Meets the state training requirements.
• Completed 75 hours of classroom and 16 hours of practical training.
• At least 18 years of age.
• Able to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable caring for the terminally ill.
• Satisfactory references from previous employers.
• Self-directed with minimal supervision.
• Calm demeanor in a patient’s home.
• Empathy for the needs of the ill and frail.

Responsibilities

• Perform personal care tasks and respond to patient requests promptly.
• Provide communication with patients and families with confidentiality.
• Assist with bathing, grooming, oral hygiene, and linen changes as needed.
• Assist in dressing, feeding, and nutrition planning.
• Monitor skin condition and prevent tissue breakdown.
• Assess patient status and report changes to the Case Manager.
• Assist with ambulation and transfers under supervision.
• Assist with light laundry and home care activities.
• Document observations and care performed.
