Job summary

CHS HospiceCare in Florida is seeking a dedicated Hospice Aide to deliver compassionate in-home and facility-based care under the supervision of a Registered Nurse or Physician. The role emphasizes accurate documentation, timely visits, and collaboration with the care team.

Applicants must hold a Florida CNA certification, CPR, and at least 1 year of relevant experience, with strong English reading/writing skills and comfort using clinical software.

Qualifications

• Valid CNA certification in Florida.
• CPR certification required.
• Minimum of 1 year of experience as a hospice aide or similar role.
• Proficient in reading, writing and understanding English; able to use computer/clinical software.

Responsibilities

• Performs personal care/home maintenance activities contained in a written assignment by a health professional employee.
• Makes visits as necessary according to assessed needs, within established time frames.
• Ensure effective communication with Interdisciplinary Teams.
• Record services provided accurately.
• Participates in Interdisciplinary Team meetings.
• Provide other duties as assigned.
