Job summary

Elara Caring is seeking a Hospice Certified Nurse Assistant/Hospice Aide to deliver compassionate home-based care in Texarkana, TX. You will follow the Plan of Care, monitor patient status, and collaborate with the RN Case Manager to update care as needed.

Ideal candidates have a HS diploma or GED, at least 1 year of hospice or nursing assistant experience, and a state-approved training program with competency evaluation. A valid driver’s license or reliable transportation is required.

Qualifications

• 1 year of Hospice Aide or Nursing Assistant experience required.
• State-approved Hospice Aide training program with competency evaluation.
• Must be able to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Provide skilled nursing care per Plan of Care.
• Monitor patient condition and report changes to RN Case Manager.
• Educate patients and caregivers on self-care techniques.
