Job summary

Elara Caring is hiring a Certified Nurse Aide to provide essential in-home care. You will assist patients with daily activities, monitor vital signs, and ensure safety in a home setting.

Responsibilities include following care plans, documenting accurately, and reporting changes to the care team. A valid CNA or Home Health Aide certification is required, with the ability to lift up to 50 pounds and reliable transportation for travel between patient homes.

Qualifications

• Current state certificate as a Home Health Aide or Certified Nursing Assistant certification.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver's license or have reliable transportation to travel between patient homes.

Responsibilities

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's individualized care plan.
• Monitor and report changes in a patient's condition to the care team.
• Maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond to emergencies and follow infection control guidelines.
• Maintain accurate documentation and protect patient privacy.
