Job summary

Elara Caring is seeking a Hospice Certified Nurse Assistant/Hospice Aide to provide compassionate, skilled care to patients in their homes. You will work under the supervision of the RN and collaborate with the care team to support safety, comfort, and quality of life.

Required: high school diploma or GED, 1 year hospice or nursing assistant experience, valid driver’s license or reliable transportation, and completion of a Hospice Aide program meeting 42 CFR standards; home health experience

Qualifications

1 year hospice or nursing assistant experience required.
Hospice Aide program meeting 42 CFR standards required.
Must be able to lift up to 50 pounds and assist with patient transfers.
Must have a valid driver’s license or reliable transportation to travel to patient homes.

Responsibilities

Provide skilled nursing care per Plan of Care and RN direction.
Monitor patient condition and report changes to RN Case Manager or care team.
Collaborate with RN Case Manager to update the Plan of Care when needed.
Educate patients and caregivers on self-care techniques for safety and independence.
Document medication regimens and maintain updated profiles.
Help prevent avoidable hospitalizations through proactive care coordination.
Maintain compassionate, high-quality communication with patients, families, and care team.
