Job summary

Elara Caring is seeking a dedicated Hospice Aide to join our Monroe County team. You will provide skilled nursing care per the Plan of Care, monitor patient well-being, and work with the RN Case Manager to adjust care plans.

You will also educate patients and families on safe self-care practices and document all medication regimens. The role requires a high school diploma or GED, 1 year of hospice or nursing assistant experience, and a state-approved hospice training program.

Qualifications

• High School Diploma or GED required.
• 1 year of Hospice Aide or Nursing Assistant experience required.
• State-established Hospice Aide training program meeting 42 CFR §484.36.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must have a valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Provide skilled nursing care according to the Plan of Care and RN directions.
• Monitor patient condition and report changes to the RN Case Manager and care team.
• Collaborate with the RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care techniques for safety and independence.
• Document medication regimens and maintain medication profiles.
• Help prevent hospitalizations through proactive, coordinated care.
• Maintain compassionate, high-quality care and communicate with patients, families, and physicians.
