Job summary

Elara Caring is seeking a Certified Nurse Aide to join our team and deliver high-quality home care. You will provide hands-on personal care under RN supervision, assist with daily activities, monitor vitals, and support patient mobility in a home setting.

Qualifications

• Current State Home Health Aide or Certified Nursing Assistant certification.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver’s license or have reliable transportation to travel between patient homes.

Responsibilities

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's individualized care plan.
• Monitor and report changes in a patient's condition to the care team.
