Job summary

Elara Caring is seeking a Hospice Certified Nurse Aide/Hospice Aide to deliver skilled nursing care in patient homes under the Plan of Care and RN direction. You will monitor patient status, report changes, and collaborate with the care team to adjust the Plan of Care.

The role requires a high school diploma or GED, at least 1 year of hospice or nursing assistant experience, and a state-approved training program.

Qualifications

• High School Diploma or GED equivalent required.
• 1 year of experience as a Hospice Aide or Nursing Assistant is required.
• State-established Hospice Aide training program that meets 42 CFR §484.36 and a competency evaluation program.

Responsibilities

• Provide skilled nursing care according to the Plan of Care, orders, and RN direction.
• Monitor patient condition and report changes to the RN Case Manager and care team.
• Collaborate with RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care to support safety and independence.
• Document medication regimens and maintain accurate profiles.
• Help prevent avoidable hospitalizations through proactive care.
• Provide compassionate care with clear communication among patients, families, physicians, and team.
