Job summary

Dignity Care Hospice is seeking a dedicated Home Health Aide to support patients at home. You will provide compassionate personal care, assist with daily activities, and help maintain patients' independence with dignity.

Responsibilities include taking vitals, aiding mobility, assisting with medications as directed, and reporting changes to the RN. Valid STNA license in Ohio and CPR certification are required.

Qualifications

• Meets training requirements per state/federal laws.
• Must have a valid STNA license in Ohio.
• 18 years of age or older.
• Self-directing with ability to work with little supervision.
• Active CPR certification.

Responsibilities

• Providing personal care including bathing, changing bed linens, and dressing.
• Preparing nutritious meals and assisting in feeding.
• Taking vitals such as temperature, pulse, respiration, and blood pressure.
• Assisting in ambulation and exercise per care plan.
• Performing range of motion and other simple procedures as needed.
• Assisting patients in self-administration of medications.
• Reporting on patients' conditions and significant changes to the RN.
