Job summary

Continuum-Care-of-Washington is seeking a compassionate Hospice Aide to join our patient-centered team in Washington state. You will work under the supervision of a registered nurse to support comfort and personal care per each patient’s plan of care.

Responsibilities include assisting with bathing, dressing, feeding, mobility, and basic housekeeping while maintaining confidentiality and documenting observations for the RN Case Manager.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill.
• Prior hospice/home health experience preferred (1 year).
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Responsibilities

• Perform personal care tasks per patient’s plan and report needs to Case Manager.
• Provide respectful communication with patient and family; maintain confidentiality.
• Ensure patient safety and use equipment properly.
• Assist with bathing, grooming, oral hygiene, and changing bed linens.
• Assist with dressing, feeding, and transfers; observe for changes in condition.
• Assist with light housekeeping, laundry, and waste duties.
