Job summary

Continuum-Care-of-Washington is seeking a compassionate Hospice Aide to support patients under the supervision of a registered nurse, delivering personal care and comfort per the patient’s plan of care.

You will perform bathing, dressing, feeding assistance, positioning, basic range of motion, and documentation in the EMR; requires a state-approved CNA program, NAC, CPR, a Food Handler permit, HS diploma or GED, and a valid driver’s license.

Qualifications

• Completion of state-approved CNA course or equivalent training.
• NAC certification current.
• CPR certification current with hands-on skills.
• Food Handlers Permit and ability to read/understand instructions.
• High school diploma or GED.

Responsibilities

• Perform personal care tasks and respond promptly to patient needs.
• Communicate with patients and families with confidentiality and respect.
• Ensure safety and proper use of equipment during care activities.
• Assist with bathing, dressing, feeding, and bed linen changes as assigned.
• Support in dressing, undressing, and mobility as directed by the RN.
• Assist with feeding while adhering to dietary guidelines.
• Monitor skin condition and report any concerns to the RN Case Manager.
• Document observations and care in the EMR system accurately.
• Assist with range of motion, transfers, and positioning as instructed.
• Provide reminder support for self-administered medications per state rules.
• Offer bedpans/urinals and light housekeeping tasks as needed.
• Report any changes in patient condition and potential safety risks.
• Participate in team activities and uphold professional standards.
