Job summary

Elara Caring is seeking a Hospice Aide to join our Monroe County coverage. This full-time role supports compassionate, in-home hospice care and contributes to the Right Care, at the Right Time, in the Right Place.

You will provide nursing support under the RN, monitor patients, educate families, document medications, and help prevent unnecessary hospital visits while maintaining strong teamwork with the care team.

Qualifications

• High School Diploma or GED required.
• 1 year of experience as a Hospice Aide or Nursing Assistant required.
• State-approved Hospice Aide training program meeting 42 CFR §484.36 and competency evaluation required.
• Ability to lift up to 50 pounds and assist with positioning or transferring.
• Must have a valid driver's license or reliable transportation to travel between patient homes.

Responsibilities

• Provide skilled nursing care per Plan of Care and RN direction.
• Monitor patient condition and promptly report changes to the RN Case Manager and care team.
• Collaborate with the RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care techniques for safety and independence.
• Document medications and maintain up-to-date medication profiles.
• Help prevent unnecessary hospitalizations through coordinated patient care.
• Maintain compassionate, high-quality care with clear communication to patients and families.
