Elara Caring is seeking a Hospice Aide for Monroe County, MI. This full-time role covers local patient homes, delivering essential in-home hospice care under RN supervision.

You will provide skilled nursing support, educate families, and help manage medications while maintaining compassionate, high-quality interactions with patients and the care team.

Formación

• High School Diploma or GED required.
• 1 year of Hospice Aide or Nursing Assistant experience required.
• State-approved Hospice Aide training program and competency evaluation.
• Ability to lift up to 50 pounds and move patients/equipment.
• Valid driver's license or reliable transportation to travel between homes.

Responsabilidades

• Provide skilled nursing care per Plan of Care, orders, and RN direction.
• Monitor patient condition and report changes to RN Case Manager and care team.
• Collaborate with RN Case Manager to update Plan of Care as needed.
• Educate patients and caregivers on self-care to support safety and independence.
• Document medication regimens and maintain medication profiles.
• Help prevent avoidable hospital visits through proactive care.
• Maintain compassionate, high-quality care with clear communication.

As a Hospice Aide., you’ll contribute to our success in the following ways:

• Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
• Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
• Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
• Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
• Document medication regimens and maintain accurate, up-to-date medication profiles.
• Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
• Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.

What is Required?

• High School Diploma or GED equivalent required
• 1 year of experience as a Hospice Aide or Nursing Assistant is required
• State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a home health environment is preferred
