Job summary

Advocate Health seeks a dedicated Home Health Aide/Nurse Assistant to support hospice patients in the Oak Brook area. You will provide compassionate personal care, assist with daily activities, and help monitor patient condition under RN direction.

Travel within the service area is required and on-call weekends/holidays are part of the schedule. Qualifications include state certification as a nurse assistant, BLS within 6 months, and a valid driver’s license.

Qualifications

• Must be certified as a nurse assistant in the state of practice.
• BLS for Healthcare Providers certification within 6 months.
• Valid driver’s license required.

Responsibilities

• Contributes to effective patient outcomes under RN guidance and plan of care.
• Performs personal care tasks and reports changes in condition to RN or manager.
• Maintains patient safety and HIPAA/privacy compliance in documentation.
• Demonstrates professionalism and supports department and organization goals.
• Adheres to regulatory requirements and participates in CQI processes.
