Job summary

Advocate Health’s Milwaukee South hospice team seeks a dedicated Home Health Aide to support terminally ill patients under RN supervision. Flexible scheduling complements a standard Mon-Fri routine with on-call weekends and holiday rotations.

Responsibilities include direct patient care, personal care, documentation, and adherence to HIPAA and regulatory standards. Requires state CNA certification, good standing, and BLS within 6 months. Travel within service area may be required.

Qualifications

• Must be certified as a nurse assistant in the state in which the teammate practices.
• Nurse assistant in good standing with the State Nurse Aide Registry with eligibility to work in home health agencies and hospices.
• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months.

Responsibilities

• Contributes to effective patient outcomes. Responsible to provide competent patient care assigned by RN and plan of care.
• Performs personal care needs including feeding patient, laundry, light cleaning of patient room and washing.
• Demonstrates effective communication skills, verbally and in written documentation, per agency, state, federal and accrediting organizations, within the assigned time frames.
• Demonstrates professionalism and works in a cooperative manner with all associates.
• Adheres to regulatory requirements and maintains annual in-service hours.
