Job summary

Oakbrook Hospice and Palliative is seeking a Weekend On-Call Nurse (RN) to administer nursing care for patients in their homes and coordinate with the IDG/team, patients, families, and referring agencies. The role requires travel within the local area and on-call weekends, with duties to ensure quality and continuity of hospice care.

Responsibilities include comprehensive assessments, care planning, patient and family education, and collaboration with care teammates.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state RN license and valid driver’s license.
• Hospice experience preferred; clinical care background required.
• Must read, write and comprehend English.

Responsibilities

• Perform initial and ongoing comprehensive nursing assessments.
• Develop and document the plan of care to ensure quality and continuity.
• Educate patients and families about end-of-life care, nutrition, and self-care.
• Coordinate with the Interdisciplinary Group/Team and referring agencies.
• Provide on-call nursing services to patients and families as assigned.
