Affinity Health Management in Brunswick, ME is seeking a compassionate hospice aide. You will work under an RN supervision to provide personal care, comfort, and essential observations per the patient’s plan of care. You will document care and report changes promptly.

Responsibilities focus on safety, dignity, and coordination with Case Managers, family members, and other caregivers; prior hospice or home health experience is preferred, and a valid CNA/NAC and CPR certification are required.

Formación

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC) credential.
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Current Food Handlers Permit.
• High school diploma or GED.

Responsabilidades

• Perform personal care tasks and promptly attend to patient requests under RN supervision.
• Provide communication with patients and families with ethical, respectful care and confidentiality.
• Maintain safety by using equipment correctly and monitoring patient condition.
• Assist with bathing, grooming, feeding, and bedding as assigned.
• Support dressing, transfers, and mobility as directed by the care team.
• Record observations and report changes to the RN Case Manager.
