Job summary

Affinity Hospice Management LLC in Wells, ME seeks a compassionate hospice aide to work under the supervision of a registered nurse, performing personal care and activities to meet the patient’s plan of care and promote comfort.

You will report observations to the RN Case Manager, assist with daily living activities, monitor safety, document care in the EMR, and support families with respect and clear communication.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Certificate as Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR (hands-on demonstration required).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and terminally ill care.
• Prior hospice/long-term care experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care tasks per patient assignment and promptly attend to requests.
• Provide positive communication with patient and family, ensuring confidentiality.
• Meet safety needs and use equipment safely (foot stools, side rails, O2).
• Give personal care: baths, back rubs, oral hygiene, shampoos, bed linen changes.
• Assist in dressing and undressing patients.
• Assist in feeding; avoid homemade foods prepared by staff.
• Provide skin care and monitor for tissue breakdown; report changes.
• Report pain or changes in condition to RN Case Manager; consider caregiver environment.
• Assist in ambulation and prescribed exercises by nurse or therapist.
• Provide ROM and transfers/positioning as directed.
• Assist with self-administered medications as allowed by state regulations.
• Offer bedpans/urinals; assist with light laundry and waste disposal.
• Perform ROM and simple procedures as ordered with competency.
• Provide respite for patient/family/caregiver when on-site, as appropriate.
• Keep living area clean and orderly; adhere to care procedures and professional conduct.
• Participate in quality assessment and performance improvement activities.
• Respect patient and family environment and personal needs.
• As mandatory reporter, report non-compliance to the Dept. of Health within 14 days.
• Report suspected abuse or neglect to proper authorities immediately.
