Choice Hospice, Inc. seeks a Hospice Aide to provide direct patient care under RN direction in patients' homes or residence.

The role emphasizes safe transfers, accurate caregiving documentation, infection control, and compassionate communication with patients and families. The position requires CNA/HHA and LTC certifications, a valid driver's license and auto insurance, CPR certification, and HomeCare HomeBase experience is preferred.

Qualifications

• Current CNA/HHA & LTC certifications.
• CPR Certification.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Aid Plan of Care.
• Assist in safe transfers and ambulation per agency policy.
• Report changes and document pertinent information and care rendered to ensure continuity of care.
• Maintain infection control principles and provide a clean, safe environment.
• Promote respectful communication with patients, families and staff.
• Respect patient privacy and property.
• Assist with proper nutrition and fluid intake.
• Support efficient operation of the agency and time management.
• Perform treatments and procedures per agency policy.
• Uphold the agency philosophy and policies to ensure quality of care.
