Job summary

Choice Health At Home is hiring a Hospice Aide in Houston, TX. You will provide direct patient care under the supervision of an RN, assist with transfers and ambulation, and help maintain a clean, safe home environment for clients receiving hospice services.

The role requires CNA/HHA and LTC certifications, CPR, and a valid driver’s license. Experience with HomeCare HomeBase is preferred, with mileage stipends and comprehensive benefits available to eligible staff.

Qualifications

• Current CNA/HHA & LTC certifications are required.
• CPR certification is required.
• HomeCare HomeBase experience is preferred.

Responsibilities

• Provide direct patient care under RN direction.
• Assist with safe transfers and ambulation.
• Report changes and document care to ensure continuity.
• Maintain infection control and a clean, safe home environment.
• Communicate respectfully with patients, families and staff.
