Job summary

LifeCare Home Health & Hospice Family seeks a Hospice Aide to assist patients in their homes under RN supervision, delivering direct care and ensuring a safe, clean living environment.

You will document health information, support transfers and mobility, and communicate clearly with patients, families, and care teams. Travel to patient homes is required as part of ongoing care delivery.

Qualifications

• Must have a HS diploma; approved CNA/Nurse Aide training program or equivalent.
• One year of hospice aide or CNA experience preferred.
• Able to read, write and verbally report clinical information clearly.

Responsibilities

• Provide direct patient care under RN supervision according to the Aide Plan of Care.
• Assist with safe transfers and ambulation per Hospice policy.
• Report changes and document care to ensure continuity of care.
• Maintain infection control and a clean, safe home environment.
• Interact with patient, family and hospice staff with respect.
