Choice Health at Home is seeking a Hospice Aide (HHA) to support patients in their home as part of our compassionate care team. The role includes direct patient care under RN supervision and collaboration with families to maintain a safe, clean, and respectful home environment.

Applicants should hold CNA/HHA and LTC certifications, CPR, a valid driver’s license, and insurance. Experience with HomeCare HomeBase is preferred, with full-time hours available at our Conroe, TX location.

Formación

• Current CNA/HHA & LTC certifications.
• Valid Driver’s License.
• Valid Auto Insurance.
• CPR Certification.
• HomeCare HomeBase experience preferred.

Responsabilidades

• Provide direct patient care to patients under direction of the RN and according to the Aid Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
• Provide necessary skill to appropriately report changes and document pertinent information and care rendered to patients to ensure continuity of care.
• Practice infection control principles to maintain a clean, safe environment.
• Promote respectful communication with patients, families, and staff.
• Assist with proper nutrition and adequate fluid intake for patients.
• Contribute to efficient operation and effective time management within the agency.
• Provide skills to perform treatments and procedures according to agency policy.
