Job summary

St. Croix Hospice in Rochester, MN is seeking a compassionate Hospice Aide (CNA) to join our interdisciplinary care team, delivering hands-on care to patients in their homes under RN supervision. You’ll support daily activities, promote comfort, dignity, and quality of life at the end of life.

This full-time role requires travel to patient homes, a current CNA certification, and a valid driver's license. The schedule is 8:00am–5:00pm with rotating weekends; we offer health/dental/vision,

Qualifications

• Current CNA certification in the state of practice.
• Valid driver’s license and clean driving record.
• Ability to travel to patient homes and provide hands-on care.

Responsibilities

• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing.
• Record vital signs and report observations to the care team.
• Assist with meal planning, preparation, and feeding as needed.
• Travel to patient homes and provide care as scheduled.
• Maintain accurate documentation of services provided.
