Job summary

St. Croix Hospice is seeking a Hospice Aide (CNA) to join our interdisciplinary care team in the Moline area. You will provide compassionate, hands-on care to patients in their homes, under RN supervision, to support comfort and quality of life at the end of life.

The role involves assisting with personal care, daily living activities, and meal support while traveling to patient homes. Current CNA certification and a valid driver's license are required, with travel a daily expectation.

Qualifications

• Current CNA certification in the state of practice.
• Valid driver's license, clean driving record, and auto insurance.
• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing.
• Record vital signs and report observations to the care team.
• Assist with meal planning, preparation, and feeding as needed.
• Promote patient comfort, dignity, and safety.
• Travel to patient homes throughout the day and provide care as scheduled.
• Maintain accurate documentation of services provided.

Responsibilities

• Deliver compassionate, hands‑on care to patients in their homes under RN supervision.
• Assist with activities of daily living to ensure comfort and quality of life.
• Record and communicate vital signs and observations to the care team.
• Assist with meals and oral care as needed while keeping patients safe.
• Document services accurately and on time for each visit.
