Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide compassionate direct patient care under the supervision of the RN and according to the Plan of Care. You will assist with safe transfers, mobility, and reporting changes to ensure continuity of care.

Qualifications

• High School Diploma or GED required or preferred.
• Unencumbered CNA license required.
• At least 1 year of Hospice Aide or Nursing Assistant experience.
• EMR experience is a plus.
• Must have a valid driver’s license and reliable transportation.

Responsabilités

• Provide direct patient care under RN direction and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes to ensure continuity of care.
