Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under the supervision of the RN and in accordance with the Plan of Care. You will assist with safe transfers, report changes, and maintain accurate care documentation to ensure continuity of care.

A CNA license and at least one year of relevant experience are required, with a valid driver's license and reliable transportation expected.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA license.
• At least one year experience as a Hospice Aide or Nursing Assistant in a care setting.
• Demonstrates interest in welfare of ill and elderly; compassionate mindset.
• Maintains confidentiality of information.
• EMR experience is a plus.
• Must have valid driver's license and reliable transportation.

Responsibilities

• Provide direct patient care under the RN and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care for continuity of care.
• Follow infection control principles.
• Communicate respectfully with patients and families and staff.
• Submit documentation timely.
• Perform other duties as required.
• Ensure compliance with laws, regulations and policies.
