Job summary

Heart to Heart Hospice is seeking a Certified Nurse Aide to join our Hospice House team in a full-time capacity. You will provide direct patient care under the guidance of the RN and plans of care, assist with safe transfers, document care, and communicate changes to ensure continuity of care.

The role requires a CNA license, at least one year of Hospice Aide or Nursing Assistant experience, a valid driver's license, and a genuine heart for compassionate care.

Qualifications

• Must have an unencumbered CNA license
• At least one year experience as Hospice Aide or Nursing Assistant
• Demonstrates compassion for ill and elderly
• Current driver's license and reliable transportation

Responsibilities

• Provide direct patient care under RN supervision per Plan of Care
• Assist with safe transfers and ambulation
• Report changes and document care provided to ensure continuity of care
• Adhere to infection control principles and policies
• Maintain respectful communication with patient, family and staff
• Submit all documentation timely
• Comply with laws, Medicare regulations and policies
