Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under RN supervision. You will assist with transfers, ambulation, and documentation to ensure continuity of care in a compassionate environment.

Eligible candidates should hold a current CNA license, have at least one year of relevant experience, and possess a valid driver's license with reliable transportation. EMR experience is a plus, and a high school diploma or GED is preferred.

Qualifications

• Current CNA license
• At least 1 year experience as Hospice Aide or Nursing Assistant
• High School Diploma or GED preferred
• Unencumbered certification according to Nurse Aide Registry
• Interest in welfare of ill and elderly; compassionate care
• EMR experience a plus
• Must have valid driver's license, reliable transportation, and auto liability insurance

Responsibilities

• Provide direct patient care to patients under the RN supervision and plan of care
• Assist with safe transfers and ambulation per policy
• Document care provided and communicate changes to ensure continuity of care
• Follow infection control principles
• Promote respectful communication with patient/family and staff
• Timely submission of documentation
• Perform other duties as required
