Job summary

Heart to Heart Hospice seeks a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of an RN. You will help with transfers, routine activities, and communicate care needs while documenting changes for continuity of care.

Applicants should have an unencumbered CNA license, a High School Diploma or GED, and at least one year of hospice or nursing assistant experience. A valid driver’s license and EMR familiarity are preferred.

Qualifications

• High School Diploma or GED preferred.
• Unencumbered CNA license is required.
• At least 1 year of hospice aide or nursing assistant experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist patient with transfers and ambulation per policy.
• Report and document changes and care provided to ensure continuity.
• Practice infection control and maintain confidentiality.
• Communicate with patient/family and colleagues respectfully.
• Submit all documentation timely and comply with laws and policies.
