Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of the RN and within the Plan of Care. You will assist with safe transfers, document care provided, and communicate changes to ensure continuity of care.

Qualifications

• High School Diploma or GED preferred.
• Must have an unencumbered CNA license.
• At least one year experience as Hospice Aide or Nursing Assistant in a healthcare setting.
• Demonstrates interest in welfare of ill and elderly; maintains confidentiality.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist patient with safe transfers and ambulation per policy.
• Report and document changes and care provided to ensure continuity of care.
• Practice infection control principles.
• Promote respectful communication with patient/family and staff.
• Submit all documentation timely.
• Perform additional duties as required and ensure compliance with laws and policies.
