Job summary

Heart to Heart Hospice is seeking a Hospice Aide (CNA) to join our team in a PRN/full-time capacity. You will provide direct patient care under the RN and follow the Plan of Care, assist with transfers and ambulation, and report changes to ensure continuity of care.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA certification/licensure.
• Current CNA license required.
• At least one year experience as Hospice Aide or Nursing Assistant in healthcare settings.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist patient with safe transfers and ambulation as per policy.
• Report changes and document care to ensure continuity of care.
• Practice infection control principles.
• Communicate with patient/family and staff respectfully.
• Timely submission of documentation.
• Perform other duties as required.
• Ensure compliance with laws and policies.
