Qualifications

• Licensed Vocational Nurse preferred and active.
• Healthcare intake experience required.
• Knowledge of hospice insurance requirements.
• 1+ years hospice experience preferred.

Responsibilities

• Receive and verify patient referrals.
• Coordinate with admission team for timely information.
• Verify patient insurance details.
• Provide admission information to doctors and admission nurses.
