Gilchrist Inova seeks a compassionate Hospice Admissions Nurse to act as the first point of contact for patients and families beginning hospice. You will complete admissions, educate families, coordinate with the care team, and support a smooth transition into hospice services.

The ideal candidate is an RN with strong clinical and communication skills, committed to patient-centered care, and able to work within a collaborative interdisciplinary team.

Qualifications

• RN licensure in Virginia or eligible in Virginia
• ADN or BSN from an accredited program
• Excellent clinical and communication skills

Responsabilités

• Assess patients and families to determine hospice eligibility and needs.
• Prepare initial care plan with Medical Director approval.
• Provide direct nursing care to maintain high-quality care.
• Coordinate with interdisciplinary team and community resources.
• Educate and support patient/caregiver education and skills.
• Arrange home equipment as needed and manage visits.
• Support families during patient death when it occurs.
• Participate in admissions projects and process improvements.
• Engage with community healthcare leaders to expand referrals.
