Job summary

Gilchrist Inova is seeking a Hospice Admissions Nurse to be the compassionate first connection for patients and families beginning their hospice journey. You will complete admissions, provide education and support, coordinate with the interdisciplinary care team, and help ensure a smooth transition into hospice services.

Qualifications

• Three years of nursing experience. Home Health or Hospice preferred.
• Virginia RN license required and current licensure.
• Strong interpersonal and communication abilities.

Responsibilities

• Completes initial patient and family assessment to determine hospice eligibility.
• Prepares and implements initial care plan with Medical Director approval.
• Delivers direct nursing care to maintain high-quality care.
• Documents findings and coordinates with care teams and community resources.
• Educates family members on care tasks, meds, injections, and vitals.
• Arranges equipment for home settings.
• Assists with end-of-life support and bereavement referrals.
• Maintains relationships with referral sources and community partners.
• Obtains consents per regulations and reimbursement policies.
