The ideal candidate will have an associate degree (preferred), 1+ year hospice experience, and strong communication and organizational skills to support a compassionate

• Associate degree preferred.
• 1+ year hospice experience.
• Excellent written and verbal communication skills.
• Strong organizational skills.
• Excellent customer service background.
• Familiarity with hospice policies and state and federal regulations.
• Ability to work with a wide variety of stakeholders.
• Effective problem-solving skills.

Responsabilidades

• As Admissions Coordinator, receive referrals and obtain critical information to ensure timely data entry in the EMR.
• Input referrals and basic patient information into the EMR to assess how hospice services may help.
• Coordinate and schedule visits with appropriate staff based on patient condition and family needs.
• Communicate with referral sources, physicians, Seasons staff, and other agencies to initiate hospice services.
• Maintain regular communication during sign-on, consent and admission assessment phases to ensure seamless coordination.
• Maintain accurate EMR documentation and referral statistics.
