Responsibilities

• Receive and process hospice referrals from hospitals, physicians, SNFs, and community sources.
• Meet with patients and families, in person or virtually, to explain hospice services and eligibility.
• Provide emotional support to families making end-of-life care decisions.
• Facilitate hospice enrollment and election processes, including required consents and documentation.
• Complete and submit admission paperwork accurately and efficiently.
• Communicate admission details with the clinical team to ensure timely start of services.
• Coordinate with intake, scheduling, and leadership teams to support transitions into care.
• Identify and escalate clinical concerns to licensed staff.
• Serve as liaison to acute care facilities and hospitals.
