Job summary

Providence at Home with Compassus is seeking an Occupational Therapist to join our hospice team, providing home-based therapy evaluations, treatment planning, and discharge planning in the West LA/Santa Monica/Hollywood area.

The therapist collaborates with the multidisciplinary team, educates patients and families, and coordinates care to ensure safe, effective home care. California OT license and BLS certification are required; home health experience is preferred.

Qualifications

• California Occupational Therapist license required upon hire.
• National Provider BLS - American Heart Association required within 30 days of hire.
• Hospice/home health experience preferred.
• Experience in home health care settings beneficial.

Responsibilities

• Evaluate patient function and safety for home use.
• Develop and communicate plan of care with multidisciplinary team.
• Educate patient/family on treatment plans and home exercises.
• Coordinate care and supervise aides and home health staff.
• Document assessments and communications accurately.
