As a Home Health Aide, you will provide personal care and assistance to patients in their homes, under the supervision of nursing staff. Responsibilities include daily living support, care plan review, and home maintenance tasks.

Qualifications

• Certified CNA in Wisconsin for work in home health agencies.
• Experience working with the public is required.
• Must have a current WI driver’s license and a reliable vehicle.

Responsibilities

• Support care by reviewing the care plan prior to providing care.
• Assist with daily activities like bathing and toileting.
• Explain care procedures to patients and involve them in care.
• Complete mandatory care upon arrival and departure.
• Engage with patients and report pertinent information to the Case Manager.
• Complete household tasks per the care plan.
