Job summary

The Employee Connect in Irving, Texas is seeking a compassionate Registered Nurse to join our home health team. This role delivers patient-centered nursing care directly in patients' homes, supporting independence and quality of life while adhering to clinical guidelines.

Responsibilities include initial and ongoing assessments, care plan development with physicians, medication administration, documentation, and coordination with case managers.

Qualifications

• Active and unrestricted Texas RN license
• Minimum of two years of clinical nursing experience
• Current CPR and Basic Life Support certification
• Valid driver's license and own vehicle
• Strong clinical assessment and critical thinking skills
• Excellent verbal and written communication abilities
• Proficiency with electronic health records and standard documentation platforms
• Ability to work independently and manage a caseload efficiently
• Commitment to patient dignity, cultural sensitivity, and ethical nursing practice

Responsibilities

• Conduct initial and ongoing nursing assessments in patients' homes
• Develop, implement, and update individualized care plans with physicians and interdisciplinary team
• Administer medications, wound care, injections per orders and protocols
• Monitor and document patient progress and vital signs
• Educate patients and families on disease management, medications, nutrition, and safety
• Coordinate with case managers and social workers for continuity of care
• Escalate clinical concerns to physicians when needed
• Maintain accurate clinical documentation in the EHR
• Engage in quality improvement and comply with regulatory requirements
