Job summary

The Employee Connect is seeking a Clinical Coordinator for our home health team in Irving, TX. This on-site role offers a stable, full-time opportunity to support patients in their homes and collaborate with a multidisciplinary team.

Responsibilities include coordinating skilled nursing/therapy services, reviewing orders, and developing care plans with regular updates based on patient status. Texas RN licensure and 2+ years in nursing are required; bilingual skills are a plus.

Qualifications

Active RN license in Texas
Minimum of two years of clinical nursing experience
Knowledge of Medicare Conditions of Participation and home health regulations
Proficiency with EMR systems
Excellent organizational, communication, and time management skills
Ability to manage multiple priorities in a deadline-driven environment
Attention to detail and accurate documentation
Bilingual EN/ES a plus but not required

Responsibilities

Coordinate scheduling and delivery of skilled nursing and therapy services for home health patients
Review physician orders, referrals, and authorizations promptly
Conduct patient assessments and assist with care plan development
Communicate with field clinicians, physicians, patients, and families
Monitor patient progress and update care plans as needed
Ensure all documentation meets Medicare/Medicaid regulations
Participate in quality assurance and performance improvement initiatives
Mentor field nursing staff on clinical questions and documentation
Collaborate with billing and admin to resolve authorization and documentation issues
Attend case conferences and multidisciplinary team discussions
