Job summary

Excelin Health is seeking a Registered Nurse (RN) for home health services in Irving, TX. The RN will assess patients, develop care plans, administer medications, and coordinate care across the team to support individuals and families in their homes.

The position requires an active Texas RN license, NLN-accredited nursing education, and strong communication with patients and families. On-site duties and potential on-call responsibilities are included.

Qualifications

• RN licensure required in the state.
• Graduate of NLN-accredited nursing program.
• Driver with insured vehicle required.
• Minimum two years’ nursing experience, public health/home care preferred.
• Able to lift and assist patients; strong mobility.

Responsibilities

• Assess patient and family to determine home care needs and perform physical assessments.
• Re-evaluate patient nursing needs regularly and adjust care plan.
• Develop and implement care plans with patient and family involvement.
• Administer medications and treatments as prescribed.
• Coordinate patient care as Case Manager when assigned.
