Job summary

Excelin Health is seeking a Registered Nurse (RN) in Irving, TX to plan, organize, and direct home health services within the community setting. The RN will assess patients and families, develop a care plan, and coordinate services with physicians and other professionals.

Responsibilities include patient assessments, ongoing re-evaluations, medication administration, and discharge planning. Strong communication and counseling skills are required to support patients and families in their homes.

Qualifications

• RN with current licensure to practice professional nursing in the state.
• Graduate of NLN-accredited school of nursing.
• Must be a licensed driver with an automobile and current insurance.
• Minimum of two years’ experience, at least one in public health or home care nursing.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs and updates the plan of care.
• Initiates the plan of care and revises as patient status changes.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing and related needs.
• Identifies discharge planning needs and coordinates care before discharge.
• Acts as Case Manager when assigned by Clinical Manager to coordinate patient care.
