Responsibilities

• Perform initial patient and family assessments to determine home care needs.
• Re-evaluate patient nursing needs regularly and adjust plans of care.
• Develop and revise care plans with patient and family involvement.
• Administer medications and treatments as prescribed by physicians.
• Counsel patients and families on nursing needs and home care.
• Provide health care instructions per plan of care.
• Coordinate discharge planning and case management for assigned caseload.
