Adoration Health in Fort Wayne, IN is seeking a dedicated Home Health Admit RN to join our patient‑centered team. You will assess patients, create home health care plans, administer medications and treatments, and provide education to families and caregivers.

This full‑time position offers a Monday–Friday visit schedule, on‑call rotation as needed, and strong growth opportunities within a growing company.

Qualifications

• RN license in the state of practice is required.
• Minimum of 1 year nursing experience in patient care settings.
• Current CPR certification is required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with physician orders and patient goals.
• Administer medications, treatments, and nursing interventions.
• Educate and support the patient’s family and caregivers.
• Maintain timely documentation and participate in on-call rotation as needed.
